Application for declaration that turnover is of low volume and low value
This form is for the purposes of Regulation 4C of the Therapeutic Goods (Charges) Regulations 1990 to declare that the turnover of the goods described is of low volume and low value.
This form can be filled in and saved to your computer using Adobe Reader version 7 or later or any version of Adobe Acrobat Standard or Professional.
